SOUTHERN AID is an African-based Non –Governmental Organization implementing several humanitarian projects including health/nutrition, livelihood and food-security programs in different locations of Somalia with the aim of alleviating poverty and fostering sustainable development practices in arid and semi-arid (ASAL) areas of the regions dominated by nomadic pastoral communities. SOUTHERN AID currently Partnered with IOM DDR (Disarmament, demobilization and reintegration Department) on Community based case management during reintegration phase of the national program on disengaged combatants for Males in Kismayo Rehabilitation Centre. We are willing to employ highly motivated and experienced person to fill the position described below.
community based case management during reintegration phase of the national program on disengaged combatants for males in Kismayo is three months program that aims to provide proper transitional plan for the graduating combatants undertaking outreach services monitoring the progress at the community level.
The Case worker will play a pivotal role in the Drop in space Center and at IOM rehabilitation through registering the beneficiaries & giving all the necessary orientations during the intake stage. The Case worker will closely work with Social Workers & act as the lead person for all case management systems of the center & will be the key person responsible for overseeing the case management process form entry to exit. This will include coordinating the various services that will constitute the rehabilitation process, linking individuals to families, communities & identifying external service providers in accordance with individual care plans with the ultimate goal of reintegrating according to defined exit criteria.
Duties and Responsibilities:
The case worker will be required to carry out;
Provide psychosocial support to the beneficiaries at the rehabilitation centre
In coordination with the centre-based social workers, develop a transition plan (from rehabilitation to reintegration) for each beneficiary one month prior to their graduation to set goals and prepare beneficiaries (and their families) for return to their communities;
Support beneficiaries in addressing challenges to livelihood goals by creating contingency plans and troubleshooting problems that arise.
Conduct bi-weekly face-to-face visits with graduates at the drop-in spaces, homes, or other agreed upon locations for up to 6 months post-graduation, and track, monitor and record progress on beneficiary case/transition plans.
Contact and follow up with graduates periodically through phone and provide informal support and monitoring as needed
Tracking, monitoring and recording beneficiaries progress
Referring beneficiaries and their families for basic support services as needed and available such as food distribution, wash servies, medical, NFIs, education and protection
Provide weekly and monthly report on beneficiaries progress
Provide information regarding the entry & exit procedures & support services available to defectors, their families & coordinate case reviews
Ensure that confidentiality & the right to privacy is maintained with regard to all beneficiaries as well as files
Maintain accurate case files including regular updates & progress of individuals against goals & objectives detailed in care plans & use information to prepare reports for the exit board.
Identify & facilitate linkages to community based support mechanisms to optimize sustainable reintegration on graduation
Provide psycho social support including guidance & basic, culturally appropriate, counselling services including screening & referral to specialist service providers as needed/appropriate.
Perform any other task assigned by the line manager
Qualification, Skills and Experience:
Diploma in Case Management, Social Sciences, Social Work, Psychology, Teaching, Healthcare or Community Development.
Fluent in Somali & working knowledge in English.
Minimum of two years of demonstrable relevant Social Development experience. Ability and willingness to work under pressure.
Knowledge of English and Somali language.
Strong communication and interpersonal skills.
Prior work experience in Jubaland State regions is highly preferred
Ability to manage a complex and diverse workload, and meet deadlines. Respectful and appropriate attitude towards the community.
Ability to contribute as team member, and working collaboratively with colleagues. Proficiency in using computer applications including Excel and MS Word
